Contact Us

This is an example of a Custom Form Page Type that is used to collect and route information requests from the website.  Once submitted, the information collected will be stored in the secure custom forms database area where it can be viewed and exported into a spreadsheet.  In addition, a Contact is created in the Contacts database area, the submitter is added to the "Submitted Contact" mailing list, an auto-responder e-mail is sent the submitter's e-mail address, and an e-mail alert is sent to the coordinator of the Contact Us Forms.

NOTE: If you notice that the fields are pre-filled, that is because some of the fields are tied ("connected") to the Contact database area, so if a logged in User views and submits this form, their Contact and My Account information will be pre-filled and updated automatically.
